    目的 了解全国医院感染现状,为制定科学、有效的医院感染预防与控制策略提供数据支持和决策依据。方法 采用床旁调查与病例调查相结合的方式,组织全国各级各类医疗机构按统一调查方案开展医院感染横断面调查。结果 2024年共调查医疗机构5 736所,调查病例2 751 765例,发现医院感染34 889例,医院感染现患率为1.27%;38 032例次,例次感染现患率为1.38%。全国不同地区医疗机构医院感染现患率为0.66%~2.35%;不同规模医疗机构中,床位数≥900张的医疗机构医院感染现患率最高,达1.65%;医院感染部位以下呼吸道最常见(44.66%),其次为泌尿道(12.94%)、手术部位(9.32%)、上呼吸道(7.02%)、血流感染(5.78%);医院感染率最高的前3位科室依次为综合重症监护病房(10.02%)、神经外科(5.51%)、血液病科(组)(5.34%)。共检出医院感染病原体23 238株,其中下呼吸道标本来源最多,为10 714株(46.10%);检出株数居前5位的依次为肺炎克雷伯菌(14.76%)、铜绿假单胞菌(13.33%)、大肠埃希菌(12.79%)、鲍曼不动杆菌(9.23%)、金黄色葡萄球菌(7.88%)。监测Ⅰ类切口手术患者231 944例,手术部位感染1 647例,手术部位感染现患率为0.71%。应送检人数(即治疗用加治疗兼预防用药人数)715 179例,实际送检480 492例,病原学送检率为67.18%;治疗前送检人数425 225例,治疗前送检率为59.46%。结论 全国医院感染总体水平较低,但在不同地区、不同规模的医疗机构间呈现不均衡态势。因此,防控工作需精准施策,重点关注高风险机构,并聚焦于高危科室、重点部位与关键环节。各级医疗机构应通过强化监测、规范抗菌药物使用及夯实基础医院感染防控措施,持续降低医院感染发病率。

    Abstract:

    Objective To understand the current situation of healthcare-associated infection (HAI) in China, provide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control. Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations. Results In 2024, a total of 5 736 medical institutions and 2 751 765 patients were surveyed. Among them, 34 889 HAI cases were identified, with a prevalence rate of 1.27%. The number of HAI episodes was 38 032, and case prevalence rate was 1.38%. The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66% to 2.35%. Among medical institutions of different scales, those with a bed capacity of ≥900 had the highest incidence of HAI, reaching 1.65%. The most common infection site was the lower respiratory tract (44.66%), followed by the urinary tract (12.94%), surgical site (9.32%), upper respiratory tract (7.02%), and bloodstream infection (5.78%). The top 3 departments with the highest HAI rates were the general intensive care unit (10.02%), department of neurosurgery (5.51%), and department (group) of hematology (5.34%). A total of 23 238 strains of HAI pathogens were detected, with 10 714 strains (46.10%) from lower respiratory tract specimens. The top 5 detected strains were Klebsiella pneumoniae (14.76%), Pseudomonas aeruginosa (13.33%), Escherichia coli (12.79%), Acinetobacter baumannii (9.23%), and Staphylococcus aureus (7.88%). 231 944 patients underwent class I incision surgery were monitored, with 1 647 cases experienced surgical site infection, and the prevalence rate of surgical site infection was 0.71%. The number of patients who should undergo pathogen detection (patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents) was 715 179, while the actual number was 480 492, with a pathogen detection rate of 67.18%. 425 225 patients received pathogenic detection before treatment, with a detection rate of 59.46%. Conclusion The overall HAI prevalence in China is lower, showing disparities among medical institutions of different regions and scales. Therefore, precise implementation of measures is necessary for HAI prevention and control, with a focus on high-risk institutions and high-risk departments, key areas, and critical procedures. All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring, standardizing the use of antimicrobial agents, and reinforcing basic HAI prevention and control measures.
